Your First Visit with Dr. Moss

We would like to take this opportunity to welcome you to our office. As a new patient, you will receive a complete history and medical examination from Dr. Moss with ample time to discuss your health concerns. This initial consultation will last approximately one hour.

He will ascertain which lab tests would be appropriate for you, and based on your current health status, if appropriate, suggest a dietary and nutritional supplement program for you to begin. Blood work that needs to be drawn in our office will be scheduled and any kits you collect at home for saliva, urine or stool testing will be fully explained by the staff. If allergy testing is required, the staff will help you schedule at your convenience. Dr. Moss may also prescribe vitamin injections and/or IV treatments, acupuncture or massage therapy which can be scheduled at your convenience.

We request that you bring your completed new patient forms with you. These forms can be downloaded from our website on this page. If you are unable to bring these forms with you, we ask that you arrive 30 minutes before your scheduled appointment time so that you can complete them in the office. Please also be sure to note on the appropriate form any prescription medications, hormones, or vitamin supplements you may currently be taking.

If you have any recent lab test results from another medical office, please bring copies of these as well. A follow-­up visit is then scheduled approximately three weeks after your initial visit, to discuss the results of your laboratory testing.  At this time, Dr. Moss will create an individualized treatment plan for you according to your personal needs.

Please call our office if you should have any questions about your first appointment. Please note, Dr. Moss is not on any PPO panels or part of any HMO organization, though many of our services are partially covered by some insurance plans.
